Registration form: Sacrament of Baptism

To register please complete the registration form below – a separate Baptism Registration Form is required for each child. After you complete your Registration Form please contact Fr Saju or one of our Deacons during their surgery to register. No appointment necessary.

Godparents

As a norm 2 Godparents are registered. Godparents must be over the age of 16 and Confirmed practising Catholics Godparents must be practising Catholics. Name and address of Parish attended by the Godparents is required.

If you are not validly married in the Catholic Church it is important that you speak to the priest about this. He will explain to you the Church’s teaching on marriage and conditions for the worthy reception of Holy Communion. He will also explain what you can do to have your marriage blessed and Solemnised by the Church. One session prior to baptism preparation exploring the Sacrament of Marriage and Christian Family life is obligatory. This should be arranged with the priest.
